I understand I am choosing to work with a fully certified and insured practitioner, "JS" of Cosmic Care Company LLC. Practitioner has received training through the International Center of Reiki Training ICRT and is a registered professional member . I understand that Reiki is a simple, gentle, hands-on or distance energy technique that is used for stress reduction, relaxation and clearing of energies. I understand that Reiki practitioners do not diagnose medical conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. I understand that Reiki does not take the place of medical care. It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have. I understand that Reiki can complement any medical or psychological care I may be receiving. I also understand that the body has the ability to heal itself and to do so, complete relaxation is often beneficial. I acknowledge that long-term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself. I understand that at all times, my personal body privacy, records and details of the session will be maintained in the strictest confidence. I am over 18 years of age. The information I have given is true to the best of my knowledge, and I have not withheld any relevant information. I agree I am fully aware of the Reiki treatment process and procedures involved. I confirm that the details given by me to the Practitioner are correct and that if any of the personal information changes, or changes between session to session, then I accept that I must inform the practitioner accordingly. I understand that if there is an emergency, a worsening of my health condition, or a new ailment or condition arises, that I should consult a licensed physician and do not seek to hold the practitioner accountable. I agree that I have had the opportunity to ask questions regarding the above, and I am willing to proceed with the treatment. I understand the fees per session are as listed on the current website (or TT). I understand and agree to the cancellation policy.
